The Resource Agencies Flood Team (RAFT) is a cooperative effort of faith-based and community based agencies to assist disaster victims as they recover. The RAFT works together to find resources to assist in recovery. Below is an application form and release of information as well as detailed information as to how the program works. Please complete these forms and return to RAFT, 1905 2nd St SE, Suite 1B, Minot ND 58701 by December 1, 2011. Include any pictures of the damage sustained, estimates for replacement or repair of damage and a copy of your 2010 income tax return (if possible). If you have insurance coverage, but still have an unmet need, it's important that you complete these forms and return them. Once your application has been received, you will be assigned a case manager who will contact you to discuss your unmet need and help you through the long term recovery process by assessing resources which you may be eligible for.
