*Must bring with you 1 express mail envelope with postage when you come in to apply. You will pay $16.25 to purchase this at the post office.

*You will pay an additional $14.85 to US Dept. of State for returning your passport to you if you want it returned by express mail.

*$14.75 pick up fee

REQUIREMENTS:
2 passport photos are required (Obtain at any photo shop)

Certified copy of Birth Certificate or previous passport unless issued under the age of 16 years old Previous U.S. passport

Certificate of Citizenship

Forms of Identification

Government employee identification card (Federal, State, Municipal)
Driver's license (not temporary or learner's permit)
Aircraft pilot's license issued by the Department of Transportation, FAA
Official military identification card

If under 16 both parents or legal guardians must accompany the child when applying. Both parents or legal guardians must sign the application for the child. If this is not possible, the single applying parent or legal guardian must show proof of sole custody or provide notarized consent of the second parent.

If printing form off website forms must be 2 single pages, also do not sign application. It must be signed here when you are in the office to apply.
