                    If you are seeking your GED you will need to write a letter to the following address:

                    The letter requesting the GED must be handwritten and contain the following information:

                                        Full Name - including maiden name if applicable

                                        Date of Birth

                                        Social Security Number

                                        Signature

                                        A check for $2.00 payable to Department of Public Instruction
